✅ What is Urethral Stricture?

Urethral stricture is a narrowing of the urethra due to scar tissue, which obstructs urine flow. It can affect men more commonly than women and may lead to serious complications if untreated.

🧬 CAUSES OF URETHRAL STRICTURE

INFECTIONS:

  • Sexually transmitted infections (STIs) – e.g., gonorrhea, chlamydia
  • Urinary tract infections (UTIs)

TRAUMA/INJURY:

  • Pelvic fracture
  • Straddle injuries (e.g., bicycle accidents)
  • Blunt perineal trauma

IATROGENIC (MEDICAL PROCEDURES):

  • Repeated catheterization
  • Endoscopic procedures
  • Radiation therapy

INFLAMMATORY CONDITIONS:

  • Lichen sclerosus (balanitis xerotica obliterans)
  • Urethritis
  • Congenital (rare): Present from birth

⚠️ SIGNS & SYMPTOMS

  • Weak or slow urine stream
  • Straining to urinate
  • Incomplete bladder emptying
  • Urinary retention or dribbling
  • Spraying of urine stream
  • Increased urinary frequency or urgency
  • Urinary tract infections (recurrent)
  • Blood in urine (hematuria) or semen
  • Painful urination (dysuria)

🧪 INVESTIGATIONS

  • Uroflowmetry- Measures the rate and pattern of urine flow
  • Post-Void Residual (PVR) Test= Checks if the bladder empties completely
  • Retrograde Urethrogram (RUG)- X-ray with contrast to visualize stricture location and length
  • Voiding Cystourethrogram (VCUG)- Assesses bladder and urethra while urinating
  • Cystoscopy- Direct visual inspection of the urethra using a scope
  • Urine tests- To rule out infection (urinalysis and culture)

MANAGEMENT AND TREATMENT

  • URETHRAL DILATION. During this procedure, a urologist uses long, thin rods that gradually increase in thickness to open the stricture. Internal urethrotomy. An internal urethrotomy involves using a cystoscope with a small knife (scalpel) at the end. A urologist uses the cystoscope to cut away scar tissue.
  • BALLOON DILATATION- The procedure of dilatation of urethra using a Balloon catheter. It is done either under local anesthesia or under short general anesthesia.
  • URETHRAL RECONSTRUCTION. A urologist will remove the stricture and reconnect the healthy ends of your urethra.
  • URETHROTOMY VIU / DVIU In this under anesthesia a cysto-urethroscope is introduced into the urethra, the fibrotic scar tissue is cut open to release the stricture. Different techniques are used including simple blade i. e. urotome, cold knife, and different lasers such as Holmium and Thulium laser.
  • URETHROPLASTY-
  • Self Catheterization / Self Dilatation / Clean Intermittent Self Catheterization (CISC)
